ABOUT CHOICES HOME CARE
Choices Home Care was established in 2006 in response to what was considered a gap in service delivery, in particular with respect to children and young adults having Autism Spectrum Disorder.
Choices currently employs 30 caregivers. The company believes that if you are naturally compassionate and have a good work ethic, our training will put you in a position to succeed. Many of Choices’ Personal Care Assistants (PCAs) work with multiple clients, as concerted efforts are made to accommodate staff work schedules. A considerable amount of time is spent matching PCA skills and schedules with the clients’ needs.

Choices Home Care strives to solve any home care situation brought to our attention. Clients require their home care providers to be flexible — to be willing to adapt to complex and changing health care needs. Choices embraces the complex: many of our clients come from other providers that quit providing services when matters became complicated.
Coordinated and coherent care plans are what separate Choices Home Care from its competition. With so many parties involved — clients, staff, family members, medical professionals and others — excellent communication is critical. Choices strives to convert all of the elements of the communication into an organized action plan that serves all parties involved.
- Consumer Clients and their Responsible Parties gain comfort from their well-prepared and well-executed care plan. The plans are designed with PCA support that allow clients to realize their full potential, and free up time for busy family members.
- Professional partners such as Case Managers and Public Health Nurses experience caseload productivity gains stemming from Choices’ ability to source PCA staff and effectively manage clients between care plan reviews.
- The PCA staff develops important caregiving skills, which serves to improve the daily work experience as well as advance their career in this growing field.

Steve Warner took over Choices Home Care, LLP in March, 2016. He is convinced of the need for expanded PCA services in West Central MN and plans to leverage his 25 years in the financial services industry to deliver a better experience for Choices’ clients and employees. Choices Home Care will invest the time and resources to identify the critical needs, and, working with our professional partners, implement the solution.
One of the most important aspects of Steve’s job is making sure that Choices Home Care is a trusted resource to professional partners such as the county case managers and public health nurses who need reliable service providers to address the growing needs for care.
Choices Home Care is also dedicated to finding the best and most efficient ways to train and retain PCA staff to deliver on the promise of delivering the most reliable and excellent consumer client experience in the industry.
